Healthcare Update for Active Children

Thank you for referring a child to Compass to Care for support.  As part of our ongoing process to meet the needs of the children we support, we ask our children’s social workers to provide regular updates on their patients medical care and transportation needs.

If you have received a request to provide an updated referral from our Program Team, this is the form you need to complete.  (If you are referring a family for the first time, this is NOT the form to complete.  Please visit: https://www.compasstocare.org/patient-referral/ )

Upon receipt, a member of our team will be in touch with you and the family for next steps in our travel support.
